Welcome! Black Oak Clinic LLC is a rural healthcare clinic owned and operated by a nurse practitioner in Broken Bow, Oklahoma. We serve and focus more on adolescent, adult and gerontological health needs. We prefer appointments scheduled ahead of time so please call, we can take walk-in's as staffing allows. We do not accept pediatric patients at this time.
We can now accept SoonerCare patients for their primary care/ medical home for adolescents on up. You can change to Black Oak Clinic LLC by calling the SoonerCare Helpline @ 800-987-7767. We are happy to be able to better serve and coordinate your healthcare!

Hours:
Mon-Tue-Thur-Friday 9am to 5pm, gone for lunch from 12pm to 1pm. On Wed we are open from 9am to noon. Please always call before attempting to walk-in.
